The Referral Process
How the Referral Process Works:
- Contact our office for a therapy-specific order form, or visit our referral forms page for some of the therapies we offer.
- Complete the form and include the patient’s insurance card and diagnosis
- Fax or email it to us at Frontoffice@infusioncare.org or 334-521-0394
Upon receipt of the referral form, we:
- Work with insurance providers to obtain pre-authorization for treatment
- Contact the patient to schedule appointment
- Assist patient with financial assistance programs if needed
- Care for the patient on infusion day
- Provide post-infusion care and infusion notes to provider

Our team handles all pre-authorizations, freeing you up to care for other patients.

We handle all infusion scheduling, accommodating each patient’s special needs.

There are a variety of financial assistance programs for patients; our team is adept at navigating those programs and communicating with patients.
